The idiom “smell something fishy” means that you suspect something is wrong, dishonest, or suspicious.

When I was little, I always read news stories about a guy who was impaled with a steel bar, or a javelin that went through his head and somehow survived. The key is not to pull it out, or he will bleed to death. Just imagine surviving but having to keep the object that went through your body — that would be very uncomfortable.

It was just a random thought.
